Our Student flu vaccine clinic will be October 29th. Posted below are the links to the permission forms. Nurse Amy will also have copies in her office. If you would like for your child to participate, please complete the permission form and return it to Nurse Amy with your student by October 28th. Written parental consent is required for your child to participate.
