You received either a phone call or an email notifying you of the following:
A P-EBT eligible day is a day that your student missed consuming a free or reduced-price meal at school because of a COVID-related absence or school closure. Springfield R-XII School District did not have a mechanism to track all COVID-related absences. The parent/guardian must attest that their student’s absence was COVID-related in order to receive P-EBT benefits. Please complete the survey at the link below to attest any absences that were COVID-related. Benefit levels are based on the number of consecutive school days the student was absent because of COVID. Weekends, holidays and other non-school days are not included.
You were provided the student's name, ID and dates of absence(s) in the message you received and will need that information to complete the survey. Please note: If your child had more than one instance of 5 or more consecutive days missed, you will receive a separate message for each absence range and must complete the survey for each date range of absences.
