Parents of student athletes, read this
If your child plans to participate this fall in middle school or high school sports, important paperwork must be completed and submitted on or before the first day of practice.
Practice begins Monday, Aug. 6, for high school fall sports, including cross country, football, girls golf, soccer, softball, boys swimming & diving, girls tennis and volleyball.
Practice begins Monday, Aug. 13, for middle school fall sports, including cross country, football and volleyball.
Please check with your child’s school regarding practice details.
Before the first day of practice
Physicals, proof of insurance, concussion agreements, and consent to treat forms are due on or before the first day of practice.
Physicals, proof of insurance and concussion agreement forms can be found on the athletic webpage. Additionally, student Citizenship Cards and Mercy Consent to Treat forms must be completed online via Home Access eSchool Parents Portal. The citizenship agreement and consent to treat forms are read-only documents that you acknowledge through Home Access/eSchool. Do not print these documents to bring to your child’s school. Your online acknowledgement is all that is needed for the citizenship agreement and Mercy consent to treat forms.
