Attention 2023-2024 Athletes: Sports Physicals on April 4th

Attention all 2023-2024 athletes.  The Cox C.A.R.E. Mobile is partnering with Glendale High School on April 4^th to provide sports physicals at no out-of-pocket cost for students with the convenience of them being at school on April 4^th.  You may take advantage of this sports physical with the completion of the parent permission form and MSHSAA-required physical document.  Please complete all attached permission forms and return them to the school nurse by April 3^rd.  Please see the school nurses for any questions.
